Stale/Broken Report Layout on Republish/Randomly

On a fairly frequent basis (due to ~100 users) I get reports that people have issues with reports, or the service in general, and I've had these issues myself.

Typically these issues are all fixed with a hard refresh (ctrl+F5) or sometimes a normal refresh. Some examples below:

- after a republish, users see the old report and the layout is broken/non-functional

- on loading a page, datahub, recent, etc the page is simply blank

I've checked with on prem IT, and it shouldn't be a local caching issue (that they're aware of).

Is anyone else having similar issues with powerbi?
