I created a report in PowerBI Service but now I need to add an image to it. Since I can't add an image in the service, I downloaded the pbix file, open it in desktop, added the image, but when I publish the report it is not overwriting the existing report in the service, it just creates a new report with the same name. What can I do to overwrite the existing report?

I wonder if you published the modified report to the same workspace where the original one is located. Please also check if there are some spaces in front of or behind the name. Based on my tests, the modified report will replace the original one if they have same name and are in the same workspace.

could you post a screenshot of the duplicated report in your workspace? It can happen that reports have the same name in a workspace, but then they have to be based on a different dataset.
Could you check, or make a screenshot from the Lineage view in the workspace, showing the two reports? There you can see the dataset/data source for the report.
One last remark, there can be two workspaces with the same name, being an old one (from an O365 group, or created by Teams), and a new one (not linked to an O365 group), which is the default now.

When you publish, you will get a question asking if you want to overwrite the file on the service.

When you hit publish, the account that you are signed into in Desktop determines the location where the .pbix file is published. So, the question is asking - are you signed into Desktop AND the Service with the exact same userid? It is possible (and happens now and then) for folks to have more than one id and when they hit publish the system publishes - but to a different location than expected. AND, if you had the other userid the first time, then it would appear in Service and not be changing - because that is not where you are publishing.
