Configuring alerts on individual person's details level

Hi Team,

I have created 3 pages, first page is summery dashboard and second is patient Details level dashboard and third page is Patient Vital info Page

source Data:-

First page:- Summary page:- Kpi :- alerts

second page:- Patient details page:-
PatientID Name       Pulserate   BP
1            John         132              105
2            Sameer     49               110

Third Page: Patient Vital info Page

Patient name :- John
Pulse rate:- 132
BP :-105
Etc.

Alerts is based on threshold values , once the any patient's pulse rate is above the 130 or below 50 in alerts section kpi count will show as no of patients & same is applicable for BP.

i need to raise an alert on dashboard level, eg. if patient name john's pulse rate is above 130 then alerts comes in the notification then user should redirect to the third page i.e. Patient Vital info Page.
