Do apps need to be manually updated every time a report changes? My clients are not seeing any changes made on the report if I don't manually click on the updated App. This doesn't make any sense. Th...

That is by design.
The reason is that a change of a report can be uploaded to the App Workspace. This will then allow others to verify and ensure that the changes are good. And then the final step is to then update the app to reflect the new changes.Whilst it is an extra step after the report has been published it is a quick update all and it is good to go!
