Possibly a dumb question, but I cannot find out where I can fix this problem, as I don't even know what to call it. I need to know why the first two columns are missing the vertical line in the header. The forth column looks correct and what I would expect. I do not recall changing any border settings.

Simple, once you know the trick. I was so focused on data, I missed the textbox container.
My 2 cents:
Right click each table cell in the header row and click "Textbox properties"
Go to the Border tab and verify the style, width, color, presets and preview.
Repeat the same to all header cells.
