Can't get connected

I can get a response that my database has been connected but I can't get data downloaded from my tables.  I am using MySql.

I have shown some screen shots.

The first screen shot shows a successful connection to my database.

The second screen shot shows an unsuccessful connection to my table.

I notice that it does not ask me for my port.  Maybe this is a factor.

I would run the query directly against the database to see if it runs (when testing, please change it to top 100).

Also, click on the "details" option shown in the second screen shot, it might show the the reason for the error.
