how to hyperlink text in table with another url field?

It sounds like you already have the columns you need in column 1 and 2. Just put them both in a table and format using conditional formatting.
EDIT: You actually only need to put the link text/description in the table, and apply conditional formatting to it by using field value and choosing URL as the field, first, ok.

Make sure the link is formatted properly:

http
https
-mailto
file
ftp
news
telnet

Based on your description, I created data to reproduce your scenario. The pbix file is attached in the end.
Table:
You may create a table visual and then go to 'Visualizations'=>'Format'=>'Conditional formatting'=>set the 'Web URL' for 'Name' as 'On'.
Finally you may click the corresponding 'Name' to go to specific link.
