I have narrowed my problem to IP ranges whitelisted by my company... working it with my IT dept which is slow and fun.

This worked for me after I disabled this setting in Salesforce.
"Salesforce session settings can block this integration. Ensure that the setting Lock sessions to the IP address from which they originated is disabled"
