PQ for Excel: data containing manual input misaligned when table is refresh

First post here. Looking for a creative solution. 

Setup:

Issue: When table is sorted and then Refreshed, the Manual Input by End User is misaligned with the person

question: Any creative solution to keep the Manual User Input aligned to the Person?

The easiest approach would be to make last field also as part of table linked to first 2 column. 

Your input table and result table (through PQ) would be different and user will be touching only input table not result table.
