I have looked at column headers, file types, formats.
I can't work out why some files combine and others do not.

Any ideas?

The issue is probably that your Transform Sample File query is pointing to a file that is not actually the Sample File. I would check the Source in the Transform Sample File, and make sure that it is pointing to the Sample File. Also, make sure that you don't expand the sample file!
