there are two limitations that make refresh fail:

  1. size of the model - 250MB at the most
  2. refresh time - cannot take more than 30 min

I was able to resolve by starting over and when selecting Salesforce Objects as my data source, NOT selecting the 'Import relationships between Salesforce tables' option. We have a fairly complicated data model so this must have been more than Power BI could deal with.
