Adding a new row under the header row with specific text in specific columns

Hello all,

This is my first post. It may be a simple task but I'm scratching head over it. 

I have a dataset imported from SurveyMonkey using GetData and it has 21 columns after transforming the data using a few steps in power query. Now as a final step I want to add a new row right next to the header row with specific text in columns 15-20. 

Any suggestion will be appreciated.
