Outlook 365 error: Last modified by 'undefined'.

I have Outlook 365 activities setup to email me when pipelines complete or fail.   Several times now I have found this error message when I know that I am the only one working in the workspace:

Last modified by 'undefined'. Since the activity was modified by another user, please sign in with your account to modify the activity.

 

Is something I am doing causing this, or is it a bug?

I am able to clear the error by clicking "change account" and re-authorizing the same account.

    I don't think I'm doing anything to cause it.  I just leave the pipeline for a few days then reopen it to find the error.
    However, I was wrong about being able to correct the issue.   The activity appears to accept my account change and validation, but it is not saved.   When I reopen the acrtivity I see the error again.

        Does this give you the information you need?  I pulled this from the JSON file.
