Hi team, please note that I am frequently facing this issue and we have an automated status reporting which is completely broken because we do not the timely refresh.

What region are you in, Power BI users with refresh node based in Australia East region might face model refresh delays now.
If it's not there, Power BI deactivates your refresh schedule after four consecutive failures or when the service detects an unrecoverable error that requires a configuration update, such as invalid or expired credentials, after certain number of failure schedule refresh will stop running.
As for why refresh failed, there may be many reasons, I suggest you look into the dataset refresh history to find the refresh error message, also you can refer to the below document:
