In the workspace list flyout, and the new horizontal tabs, I have noticed that the workspace images disappear sometimes, and fall back to the default (generic) workspace icons - as if we hadn't uploaded any workspace image.
Later, the images re-appear. Refreshing the browser window usually brings them back.
This only affects some workspaces at a time, not all workspaces are affected at the same time.

This is an issue for us because we use the images to visually confirm whether an opened item belongs to Dev, Test or Prod.
When the images disappear, the risk of making changes directly in a Test or Prod item increases greatly.
