Merge Database Name screen and Authentication Screen into one so that the user interaction is consistent

I could connect to SQL Anywhere 16 database and use Get Data Effectively. After the August update I face problems because the wizard screens do not show up consistently. To connect to data presently two screens are used, one to specify the server and database and the second for authentication. Recently I have seen the second screen is side-stepped and the screen that follows the first screen displays inconsistent and often erroneous information. Of course the connection is not made.
