Multiple variable in y axis, timeperiod in x axis and values to show as trend - which chart to use

Hi

 

I need to present mutplie variables/categoris name in Y axis , timeperiod in X axis and values for each category as trend, please suggest the chart and whether it is possible?

Thanks for replying, i am looking for displaying variable names in Y-axis, the variable can vary from 3-8 variable. We should be able to customzie each line with iconsPatient Profile.png
