Allowing user to answer questions in Power BI

Hello,

 

I am currently trying to create like a suggestions box within a power bi report. What i would like to see is a place where it list the questions and then has a place for them to either select or write in the answer to the question. For example a question would be "Store Manager Email" and then below it would allow them to type it in. Once all questions are answered there would be a submit button where it would then send to an underlying data set or excel file for storage and reset all the questions for the next entry. Is there something out there that would allow me to do so. I looked into microsoft forms but the place they will be using does not have good internet connection.
