flowchart to follow patients through hospital

I am looking for a flowchart to follow patients through a hospital. 

I have tried to use the Sankey visual and come a long way, but not exactly there where i want to be.

What i would like to get is something like this:

does anyone now a visual that helps me?

We have seen examples of SVG based charts that do this, for example inside the HTML5 visual.

What insights are you trying to get out of that data?
