    Hello rashmi_r04 ,

in general you cannot just embed things into a Power BI report.

It's not HTML, so you have to rely on the possibilities of Power BI.

There is a Power App visual, so you could embed a Power App and realize a questionnaire then. Otherwise you can always put a link to an external website in Power BI.

If you need any help please let me know.
If I answered your question I would be happy if you could mark my post as a solution ✔️ and give it a thumbs up 👍

    Best regards

Hey rashmi_r04 ,

could you solve that with a PowerApp or a normal link?

I'm curious if you could solve this issue 🙂
