USERELATIONSHIP and Or Clause in Measure

Hello Expert,

here is my model.  Trials connect to d_date on On_Trial_Date (active) and Protocol_Open_to_Accrual_DT (inactive).  I'm trying to count distinct protocol base on Protocol_Open_to_Accrual_DT.  

my report filters (all filter select defualt to All)

CAL_YEAR slicer base on D_Date[CAL_YEAR]

I created 2 measures:

The result of this calculation gave me 121 protocol.  However, when I run the code below in SQL tool, I got 431 protocol.

Do you see any issue with my measures (syntax or structure)?  Your help is greatly appreciated.
