Star Schema on Customer Survey Dataset

Hi, Has any one built a star schema on Survey Datasets, I wonder how you built the model on multiple choice questions. If customer answered multiple choice question does that mean we have to built Many to Many Relationship.  if anyone could share a sample schema model, it would very much appreciated . thanks

But it was laid out like this:

 

ReponseID,Question1Response,Question2Response,Question3Reponse,Question4Reponse ...

 

There is a version of this where you unpivot the questions but we didn't do that.

Thanks I got you. we have similar flat file stricture at source. But this survey is repeatable so it is better we do it in star schema. Also transposing the data to rows helps as the number of questions can reach up to 40.
