Patch gallery items to SharePoint list

Hi everyone,

 

I have a gallery with 5 labels and a TextInput field with a button. 

Each time you press submit, I want to be able to create a new line item in my SharePoint list called Contacts, that contains all five labels plus the text that was entered into the input box.

 

What is the easiest way to achieve this? Any help is appreciated!
