Does anyone know how to create subheadings on matrix columns e.g.
Where Actual, Forecast, etc are measures. Preferably without having to duplicate data as this is already a huge dataset.
Or maybe a custom visual that can achieve this?

According to your requests, which may not be possible. But you can still consider my suggestion:
1.1 Create a column to realize, please see the 'Column1' in the table:
1.2 Use 'Drill' feature of Power BI Desktop:
2.1 I kow your subtitle are measures, if you use measures, the effect may not be the same as you think:

I agree that it's probably not possible with measures. But making a seperate calculated summary table and using that for the visual in your first method may work. I'll give it a shot.
