LOOKUPVALUE Randomly Returns Blank Values for same value

Hi all,

I got the issue with LOOKUPVALUE, some row returned value as blank for the same value. I have revierwed again the relationship but everthing looks good, I would aprrciate your help to fix this error. Thank you
