Identifying and counting combinations

Hello everyone,

 

I have data with two different drug types and and im trying to identify which drug combinations appear most frequently. However most cases have multiple drugs so I am wondering if there is a way to do the transformation below, and once complete, how to count which combinations appear most frequently.

 

Starting Data:
