How to create a new table using another table but have the date 6 months ahead

Hello, I want to create a new table based off another one, grabbing all the same data but for the date I want it to be 6 months ahead and if possible, have the ID have a "-ND" added to the end of it. Example added below of the desired output of the new table. Any help is appreciated. Thank you!
