Flatten dimension table

  Hi All,   I have this below table ,Level 1, level 2 and level 3 have only one value repeated. rest all levels have different data .   Can you please help me to flatten this table ...

Hi Anonymous ,

 

I am still struggling to understand what you are trying to achieve here. 
I understand that you are trying to identify rows as Unique rows using the index column, but I don't understand the part what is not working.

Can you explain more?
