If you saved your pbix just locally then the only one that got updated was your local copy. You need to re-publish it for the changes to reflect in the service.
If you made the changes in the online copy of your report, saving it will suffice. No need to re-publish.
And if I do the above, will the shareable link and access/permissions remain the same? - You are overwriting an existing report so the link and access/permissions should remain the same.
