Export a list containing: Report Page Name, Visual Name, and Source Table and Column Name

Replying to myself... another way to think of or solve this would be:
For a given source table, get a report out of everywhere columns in that source table are used.
The output table would be the same as my original post, but with fewer rows since it's only reporting on one data source, not all of the data sources used in visuals.
