Errors after pivoting column

Hi!

So I have two columns "Custom Field Name" & "Custom Field Value", I want to have a column under each "Custom Field Name" category and being represented by the "Custom Field Value" data. The problem I'm having is that once I perform a pivot on the first column a lot of errors start to arise (Please see picture #2). I've tried creating an index column and it removes errors however it creates a new issue which is that the table becomes a lot larger and the values on the "Custom Field Value" column are not consistent (The value appears only some rows) [ Please see picture #3).
