I've done this quite a few times. In my previous company, we used MS Form for our QA process. The QA used the form to answer questions related to item being audited. The answers are automatically logged into an Excel online file which Power BI connects to. The form's output still had to be transformed with Power Query before loading into the model as it wasn't in the best format for reporting.
