Create a table with a current date column as header with conditions

I need to achieve this:

Customer in first Col; Month/Year as major header; WAP (average price), Quantity and Sales as the minor header

At present , My BI data is in this format

How can i have a date header and then do a sumif formula so that I am able to sum-up the quantity and sales for a particular customer in a given month.
