Create Power BI Reports Feedback Form in Power BI Reports to populate data into SQL Server or SPoint

Hi All,

I am in middle of creating a Power BI Design Template in Power BI Desktop which will have certain guidelines to be followed by other PBI Report Creators/Authors across my organization to maintain consistency in the Look & Feel of the reports.

I am also planning to incorporate a 'Report Design and Functionality Feedback Form' in the last page of this template so that Report Users/Viewers can enter their name, designation, report and/or page name, visual name, what is wrong or could be improved with the report or visual, should the frequency of data refresh be changed and other such details.

I want the Report User to access all his reports and then got to the last page of the PBI Report and fill in the form and submit, and the data can go and sit in a database (we have MS SQL Server and SharePoint Online, can use SharePoint List or Excel), when a row is added, it should trigger an email to be sent out to the Business Intelligence team so that the request can be actioned accordingly. I am also looking to create a Power BI Report based on this Feedback Data from End Users for the use of BI Manager. 

What is the best way to achieve this?

I tried to copy and embed Microsoft Forms link but it doesn't work!

Can I use the PowerApps Visual instead? If so, how?

Any leads or step-by-step guides are greatly appreciated.

I am afraid it is not support, you can not use power bi reports to input data and save back to database. Power bi is a platform to create visualized reports based on existing database and share them to others business decision.

How about being able to use and leverage on PowerApps visual in Power BI Desktop and embed a Microsoft Form that will feed MS SQL Server directly and then being able to report on the same data in Power BI?
