Cleaning up Data

Good morning, 

I am new to Microsoft BI and I am trying to clean up my data as it is messing with numbers when trying to create a dashboard. I have a report for exams and on the report it has columns for name, quiz name then a column for each question. For example: 

Jane Doe has 5 rows under name, then under Quiz Name the quiz is listed 5 times, then under question column is the name of each question. How do I merge data so when I make a dashboard it shows her as only one person per exam and not 5 times?  Thanks!

The most important parts are:
1. Sample data as text, use the table tool in the editing bar
2. Expected output from sample data
3. Explanation in words of how to get from 1. to 2.
