Can't find the right Visual

I am trying to create a chart that will allow for dates on a top axis, percent on the x axis and Names on the y axis.  I am looking to create something along these lines that will also allow for conditional formatting of the data bar to change color at a set threshhold. I have not been successful thus far and any help is much appreciated.
