Calculate billable rate

Hi,

I want to calculate billable hours from the below table: 

And then be able to create a pivot table being able to slice and dice, showing billable rate per Type, Individual, per month and year respectively, as below (I have filled in a few numbers to exemplify, hope it helps to clarify). Please note there will be months where some individuals have no time reported and that needs to be considered when calculating billable rate on year total level.
