The data source for this report is provided by the The Centers for Medicare & Medicaid Services (CMS), a federal agency that runs the Medicare program. Please click here for further information. The purpose of this report is to show the average readmission rates across the US for patients who have had a heart attack or heart failure. The benchmark indicator is compared to the national rate (benchmark) this indicator compares the different hospital facilities and how they compare to the national rate (i.e. Above, Below or On Target) as compared to the national rate.
According to the CMS:
"Information on hospital return days helps you evaluate a hospital’s quality of care based on how much time patients spend back in the hospital during the first 30 days after discharge. For example, hospitals with fewer than average days are likely arranging timely follow-up, ensuring that patients have the proper medicines, and releasing patients into a proper aftercare location"
