Outlook Step for email on Failure didn't activate after step failed

Hi - looking at my pipeline, I have an Outlook step to notify me via email on Failure at any step in my pipeline if it fails. 

Last night the pipeline failed on the first step as seen in the screenshot but the email was not triggered. Any idea why? Can i not have multiple inputs to the same step like I have?

Hi,

The connections between the activities work like an AND operator. In your case, this means that all four activities should fail in order to get the Email Alert activity triggered.
