You would need to use another column or measure as a tie breaker. The TOPN function accepts multiple ranking parameters, so you just need to decide how you want to determine what is in the top 3 - alphabetical, most recent, earliest etc. If you don't have a column which uniquely defines a row then you could add an index column using Power Query.
