Auto populating the comments and recommendations on student report

Hi, 

Being a modest user of DAX, I have been caught relatively small issue in DAX expression. I have a student performance table, where I need to comment and recommend actions based on student performance. These comments and recommendations should see the two conditions at least like [CLO] and [OBE Band] and fetch the comments and actions from the corresponding columns of the table. Having a pivot model and physical relations between the RActions (Dimension Table) and Final Table(Fact Table), I tried to regenerate the sample that is required.

I need to use just DAX measure as I have solved the solution in Power Query Table.
