Count Function Giving Incorrect Results

I want to create a visual that shows number of items per month. I have created a Slicer for Year & Month. But When I am using this :

Kamana08 Are all these columns in the same table? What does your data look like? Can you post an example?

The most important parts are:
1. Sample data as text, use the table tool in the editing bar
2. Expected output from sample data
3. Explanation in words of how to get from 1. to 2.My Data is something like this, My Expected Output is:
In Slicer when I select Months, then it should show number of projects for that month
Example:
for Feb : 2
March : 3
April : 5
