PhARMDToolUtilizationPatientDetails provider switch
Hello!
I am a provider who recently moved facilities. Can someone switch me over on the CDW data pull for the PhARMD Tool? I utilize this so I can have 100% PharmD tool utilization on my eligible notes and this identifies the patient and note that it may have been missed. Posting here as a hail mary if anybody can fix.
